Falcons LAX financial Agreement
New Player: This is for your parents. It tells them how much it cost and forms of payment. We have multiple payment plans, so let your parents decide.
New Player: This is the uniform order. Think of a football uniform for sizing, you’ll be wearing shoulder pads under the jersey.
New player: This is for getting the stick, helmet etc. Talk to the other players about it and try to avoid buying cheap stuff from the local sport stores. In this game your equipment is very important for protection and ball handling.
New player: Fill this one out so we know how to get a hold of you. However this web site is the main communication forum.
New Player: This form HAS TO BE completed by your doctor before you can participate in practice and/or games, no exceptions.
New Player: We typically have a medical volunteer at the games. However, we need this form if you get hurt and need to seek medical attention. It does not happen very often and most head traumas, open fractures, nose bleeds and emergencies are normally treated with Ibuprofen and Tylenol on site……. 
New Player: This is your contribution to the team. Lacrosse is expensive and seems like we never have enough money to cover the expenses. The adds are affordable and easy to sell. If we get each player to sell a $50 add, we’ll raise over $1000 for the team.
